In order for you to receive Preschool For All (PFA) funding we are required to have you complete the Family Agreement Tuition Credit, and Data Collection Acknowledgement and Consent forms below. Please complete the forms and submit them along with your proof of San Francisco residency to: info@growingplacefamilypreschool.org BEFORE your child begins school.

Please make sure you:
Submit proof of SF Residency (We already have proof of your child’s date of birth from the Physician’s Report on file.)
On page 2, choose your Tuition Donation Option (donating an amount of the credit to the school vs reducing your own tuition with the full PFA credit amount)
On page 2, the PFA credit is connected to the hours of 9 a.m. to 1:30 p.m. which is reflected in the tuition amount listed on the form. Your actual tuition installments will reflect your child’s schedule.
